Remember:
General advice leaflets are available from the Surgery, including the following:- High Blood Pressure and smoking, Head Lice, etc.
The NHS does not pay for some of the services we provide. These include private sick notes, insurance forms, holiday cancellation forms, medical reports, fitness to travel certificates, private prescriptions, passport signing and some vaccination services. Our fees for these services, which are in line with BMA approved national guidelines, are displayed in the surgery.
You only need a doctor’s note if you are unable to work and are ill for longer than seven calendar days. Your employer will provide you with an SC2 self-certificate form for shorter periods of illness.
If a Private Certificate is required there is a charge for this service.
Please telephone the surgery AFTER 12 O’CLOCK to check if the results of your tests are back. Results are never given out over the phone. The Doctor or Nurses will provide this information in a confidential manner.

Since our telephones are very busy in the early morning, we ask you to telephone for test results (eg blood, urine, x-ray) between 11 and 12.30 each day.
