My Health Online is a web-based service provided by the Welsh health authority to allow patients to book and manage appointments and prescriptions. You will need to complete the respective application forms (links to which are on this page) and return them to the surgery. Proof of id is required.

Once set up you can login via the link provided and use the service.  Our practice ID is w97063.

Patient Information

We currently have an open list and welcome requests for registration from patients living in or moving to the practice area.

In order to complete the registration we require the complete form with:-

  • NHS medical card or
  • Birth Certificate or passport and
  • An up to date proof of address

We recommend that new patients undertake a health check with a practice nurse.

Please ask for a health questionnaire, which is also available for download here.
Completing this ensures that your health records or up to date and
maximizes your use of the practice services.

Test ResultsPlease telephone the surgery AFTER 12 O’CLOCK to check if the results of your tests are back.
Sick CertificatesYou only need a doctor’s note if you are unable to work and are ill for longer than seven calendar days.

Disabled access

There is good access for the disabled to all of our consultation rooms. The surgery has a ramp and wide doors to allow for wheelchair access. We also have a disabled toilet.

We also have an Induction Loop and Minicom at the practice for the hard of hearing. Please tell the Receptionist if your hearing or eyesight is impaired.
